HOW TO SUBMIT A CLAIM?

Follow these steps:

1.

Complete the claim form

2.

Complete an Agreement of Loss form (if total claim is in excess of R 25 000) and remember to include the following information: 

  • Client name and policy number
  • Name of the doctor you are claiming for and the service date
  • The amount claimed
  • The amount that your medical scheme has paid and the balance outstanding less any excess amounts that need to be paid to you
  • Your signature at the bottom of the form

3.

All accounts relating to the procedure must be attached and be submitted at the same time. 

4.

Attach a copy of the medical scheme statement indicating scheme payment towards the claim

Submit a copy of your proof of banking details
