HOW TO SUBMIT A CLAIM?
Follow these steps:
Complete the claim form
Complete an Agreement of Loss form (if total claim is in excess of R 25 000) and remember to include the following information:
Client name and policy number
Name of the doctor you are claiming for and the service date
The amount claimed
The amount that your medical scheme has paid and the balance outstanding less any excess amounts that need to be paid to you
Your signature at the bottom of the form
All accounts relating to the procedure must be attached and be submitted at the same time.
Attach a copy of the medical scheme statement indicating scheme payment towards the claim
